Reminder: Classification Changes
Health & Welfare Extended Reserve Classification Change:
Please be advised that the Health & Welfare Extended Reserve Plan is having open enrollment during the month of November. Please note that the current classifications are $0.00, $1.00, $2.00, $4.00 per hour.
All approved classification changes will be effective January 1, 2019.
All completed forms must be received in the Union Office by November 30, 2018.
You can call the Union office to request that forms be mailed or emailed to you or you may also stop by during normal business hours to either pick up forms or complete them at the office. In addition, you have the option to sign and email the forms back to us at openenrollment@local393.org.
401 Pension Classification Change:
Changes to the voluntary contribution to the 401 Defined Contribution Pension can be
made any time during the year, with a maximum of three (3) times per year.
Please note that the current classifications are $0.00, $1.00, $2.00, $4.00, $6.00, $8.00, $10.00 and $12.00 per hour.
You can call the Union office to request that forms be mailed or emailed to you or you may also stop by during normal business hours to either pick up forms or complete them at the office. In addition, you have the option to sign and email the forms back to us at openenrollment@local393.org.
